- Paid collaborating or visiting artists, contracted professionals, scholars, performers, guest lecturer, jurors, etc. are allowed to visit campus for academic and research activities with prior approval. Be mindful of infection rates and strongly consider hosting remotely before approval is sought.
- Based on the Executive Order No. 13G (EO13G) recently issued by the Governor regarding vaccinations for certain visitors to campus, guest visitors or contracted professionals may be subject to completion of the Contractor’s Declaration Attesting to Compliance with Executive Order No. 13G form.
- Applicability of the EO13G and collection of the attestation forms will be determined and administered by the Dean’s Office and University Business Services according to the answers given to the questions below.
- If the visit meets the eligibility of the EO13G, signed attestation forms will need to be received prior to when the on-campus activity/event occurs.
- Please plan accordingly and submit requests at least two weeks in advance of the proposed visit. The Dean’s Office will email the Requestor to confirm status.
